It was supposed to be a lighthearted moment — a former president grabbing his favorite ice cream in his beloved Delaware beach town. Instead, it left Americans worried.

Joe Biden, now 82, was spotted over the weekend at Double Dippers in Rehoboth Beach. Wearing a blue polo, white shorts, and a presidential cap, the visibly thinner ex-president smiled for photos with shop owner Eluh Diehl. His order? A large waffle cone loaded with vanilla soft serve.

But behind the smiles, his frail frame and gaunt cheeks were impossible to miss. “He’s lost a ton of weight,” one local told us. “It’s shocking. He used to have more energy — now he looks like a shell of his former self.”

Social media lit up after the photos surfaced. One commenter wrote bluntly: “Man looks like a literal skeleton.” Another added: “I hope his treatment and recovery are going well — but it’s clear he’s struggling.”

Biden has rarely been seen in public since announcing in May 2025 that he’s battling an aggressive, bone-metastasized prostate cancer with a Gleason score of 9 — one of the highest severity ratings.
While hormone therapy can slow the disease, his diagnosis was devastating.

Gone are the days of a massive post-presidential entourage. Instead, sources say Biden now has just “one or two aides” and a minimal Secret Service presence. Most days are spent at his Delaware home working with a ghostwriter on his memoir.

“He’s very in the wild now,” said a longtime friend. “No motorcades, no private jets. Sometimes he even takes Amtrak or commercial flights. People call him ‘American Airlines Joe’ — but it’s more about necessity than charm these days.”

Medical experts say Biden’s cancer may have been developing for years before his May announcement.
“He probably had it while he was president — maybe for a decade,” said Dr. Zeke Emanuel, a top oncologist and former Obama health advisor. “It’s the kind of cancer that grows slowly but quietly spreads. By the time it’s found in the bones, it’s serious.”

Biden’s diagnosis came after a routine exam revealed a prostate nodule. His team says he and Jill Biden reviewed “every possible treatment” before choosing his current course.

In an emotional Instagram post following his diagnosis, Biden wrote:
“Cancer touches us all. Jill and I have learned we are strongest in the broken places. Thank you for lifting us up with love and support.”

But those close to him say the strain is real.
“He’s trying to put on a brave face,” said a source familiar with his daily routine. “But the fight is taking its toll. You can see it in his eyes.”

Even as President Donald Trump charges ahead with his second term and the 2026 midterms loom, his once-fierce political rival now faces a battle of a different kind — one that no election can win.
