Disturbing New Details in Death of Mother and Daughter at Hotel

A newly released medical examiner’s report is revealing disturbing new details about the deaths of Tawnia McGeehan and her 11-year-old daughter, Addi Smith, who were found dead in a Las Vegas hotel room while in town for a cheer competition.

Authorities previously said the case was being investigated as a murder-suicide.

According to the Clark County Medical Examiner’s report obtained by TMZ, both McGeehan and Addi died from gunshot wounds to the head.

The report also revealed that McGeehan had several medications in her system at the time of the deaths.

The tragedy unfolded in February 2026 after McGeehan’s ex-husband became concerned when he could not reach either her or Addi and called police to request a welfare check.

Staff at the Rio Hotel went to the room and reportedly found a chilling note outside the door.

The note warned, “Call 911 before walking in. Two dead bodies and one is an 11-year-old.”

Hotel employees then entered the room and found McGeehan and Addi lying in a bed.

Police arrived and discovered two suicide notes inside the room.

According to the report, one of the notes said Addi had been “heavily medicated” beforehand.

Investigators also found several prescription medication bottles belonging to McGeehan, along with an open package of THC cannabis gummies and bottles of over-the-counter sleep aids.

Near a trash can, officers reportedly discovered what appeared to be broken and mostly empty gabapentin capsules, as well as a clear cup containing a white residue.

Police also found cartridge casings on the floor and a Ruger handgun inside the room.

Toxicology testing showed that McGeehan had medications in her system commonly used to treat depression, insomnia and anxiety, according to the medical examiner’s report.

The report also noted that McGeehan had a history of severe depression and had previously been prescribed antidepressant medication.

Family members or others familiar with her condition reportedly told investigators they knew she had experienced suicidal thoughts in the past, but they were not aware of any previous suicide attempts.

One officer also noted that McGeehan had reportedly been severely depressed over relationship breakups and an ongoing custody battle with her ex-husband.

The deaths stunned those connected to Addi, who had traveled to Las Vegas for a cheer competition.

The medical examiner’s findings now provide a clearer picture of what investigators found inside the hotel room, including the medication, notes, firearm and toxicology results, but the circumstances surrounding Addi’s death remain especially heartbreaking.
